combined Supply - Initial Application

This application form is to be completed by the licensee and submitted to Dee Valley Water (the Company) at the initial application stage of the combined supply application process. This stage of the process is referred to as Stage 2 in section 2.3.1 of the Company’s access code.

The purpose of this application form is for the licensee to provide the Company with relevant background information to enable the Company to make an initial assessment of the licensee’s proposal. Further information might be required in order to make the initial assessment.

If the application proceeds to the detailed application stage (Stage 3), a second application form requesting more detailed information will have to be submitted by the licensee.

The Company will only respond to a licensee’s submission of this application form if the licensee has confirmed that it will pay the Company’s application fees and a confidentiality agreement has been signed jointly.

Specify which of the following types of access is required:

• potable water into supply system
• non-potable water into supply system
• raw water into raw water main or treatment works.

• ten figure OS grid reference
• whether the location is a current point of entry.

If unsure of the point of entry, give location of the source of water.

• ten figure OS grid reference or post code
• whether the location is a current point of exit
• customer’s name, address and nature of business.

If unsure of the point of exit, give location of the premises to be supplied.

• the proposed or actual commencement date
• any proposed termination date.

• name and type of water source
• whether source is ground, surface or impounded water
• any catchment risk assessment
• any available quality information to enable the suitability of and likely risks associated with the water to be assessed (include reference to cryptosporidium)
• whether the intended source is currently utilised by the Company
• deployable output of the source (include how ascertained and any restrictions that might apply).

• proposed flow profile at the point of entry to the supply system
• proposed flow profile at the point of exit from the supply system (including peak hour, peak day, peak week, average and minimum flows in conjunction with any information relating to diurnal, weekly and annual variations)
• proposal for measuring flows and flow profile at the point of exit
• any tariff optimisation assumptions (pumping regimes) or assumed effects of storage.

For a proposal to introduce treated water, provide a summary of treated water quality.

• evidence that the quality of the water is and will remain suitable for entry to the supply system and customer use
• water quality monitoring facilities in place or proposed
• contingency plans in place or proposed in the event of the supply failing output quantity and/or quality parameters.
